*Dr. Sorensen teaches at Roseman University College of Dental Medicine on Tuesdays & Thursdays.

Your First Visit

Please arrive 10-15 minutes early to fill out your new patient packet. Except in emergency cases, you will be seen first by our licensed dental hygienist. Digital x-rays will be taken and your teeth will be cleaned gently and meticulously. Following your cleaning, Dr. Sorensen will meet with you and examine your teeth and mouth thoroughly. If you require additional treatment, Dr. Sorensen will personally discuss your options with you. Occasionally, treatment can be completed on the same day as your first visit, however, a complex treatment plan will require an additional appointment(s) to provide treatment. Our entire staff looks forward to partnering with you in optimizing your dental health.

Please assist us by providing the following information at the time of your consultation:

  • Your dental insurance information (primary and secondary insurance policies, name of policy holder, group name/number, etc.)
  • Your dental/medical history
  • A complete list of medications you are presently taking

Please alert the office if you have a medical condition that may be of concern prior to surgery (diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, allergies, etc.), or if you are on any medication (heart medications, aspirin, anticoagulant therapy, etc.), or require medication prior to dental cleanings (antibiotic prophylaxis).

IMPORTANT: A parent or legal guardian must accompany all patients under the age of 18 at the initial visit.

Financial Arrangements

In order to provide the highest quality care at the most reasonable fees, financial arrangements must be made in advance of treatment. Payment is expected at the time of service. For your convenience, we accept  Visa, MasterCard, American Express and Discover. We also offer no-interest financing through Care Credit.

Dr. Sorensen is a preferred provider on most dental insurance plans. Unfortunately, we do not accept Medicaid.  Insured patients are expected to pay deductibles and co-pays at time of service.

As a courtesy, we will bill your insurance company on your behalf. Please understand that you are personally responsible for any payment denied or refused by your insurance company.
